2017 Equipment Distribution / Physicals Day
UPDATED SCHEDULE FOR 7/22:

9am – Jr. Pee Wee
10am – Mitey Mites
11am – Flag
On July 22, 2017 we will be hosting our annual Equipment Distribution Day at Arcadia High School. We will also have Dr. Antonio Zamorano and Carlos Angulo and team administering Physicals for $20.
Only an official 2017 Pop Warner Physical Form (printed in color) will be accepted. No exceptions.
Also plan to bring extra Money $$ – vendors will be onsite selling Tri City Saints fan/athletic gear to help you get ready for the season.
Your Head Coach will be reaching out with more information regarding what to bring, exact times and scheduling.
Please note: All fees must be paid in full prior to receiving any equipment. No exceptions. We will have our treasurer on site taking payments on this day.

- 2017 Physical Form – Click here to download. All participants must use this form. No other form will be accepted and must be printed in FULL COLOR.
- Original Birth Certificate
- Report Card (most recent/latest). If possible, please provide 3 copies.
Please note: If your child excels in the classroom (96 percentile or above) and you have a FULL YEAR’S worth of report cards to show for it, your child is eligible for National recognition and scholarship. Inquire with your team PA to get more details for this process.
